Living Well Chronic Disease Self-Management Program
PIKE COUNTY ― The Pike County Area Agency on Aging’s Health & Wellness Program will offer the Living Well, Chronic Disease Self-Management Program at the Blooming Grove Senior Center on Thursdays starting March 19th and running through April 22, 2020 from 1:30 p.m. to 4:00 p.m. This is a 2 ½ hour program one time per week for six weeks.
A light snack will be provided each week. There is no cost for Pike County senior citizens to participate. Stanford University developed this course to give older adults with chronic diseases―such as high blood pressure, diabetes and heart conditions―tips to help them make their daily lives more enjoyable. The program teaches skills to manage their conditions and build their self-confidence so they can successfully adopt healthy behavior, improve communications with their physician, and enhance their quality of life.
Topics include: managing symptoms, fitness/exercise, nutrition, communication, medications, working with health care professionals and systems, and more. This program is recommended by the Administration on Aging, the principal agency of the U.S Department of Health and Human Services.
